Primary Care Networks (PCNs) bring GP practices together with other local services and help join up services at a local level, focusing on the specific needs of these local populations, with patients still accessing routine GP appointments as they do now.
All 5 of the GP practices within our area have joined together to form the South Hams PCN and we are looking to build on current primary care services and enable greater provision of proactive, personalised, coordinated and more integrated health and social care. Our clinicians describe this as a change from reactively providing appointments to proactively providing care for the people and communities they serve. Working as a PCN provides clear benefits to patients through providing access to clinicians such as Pharmacists, Physiotherapists, Social Prescribers, Paramedics, and Mental Health Practitioners that an individual practice would not be able to offer.
